**FY2020-21 HEALTH PROMOTION PROGRAM**

  • Its time to STOP and think about your health…

    Welcome to the 2020/2021 Health Promotion Program! Participation in the program is voluntary, and the goal of the program is to help you establish a relationship with a Primary Care Physician, improve and maintain your health, and keep your health benefits affordable and sustainable.

    In order to qualify for the Platinum Health Plan for next fiscal year (7/1/2021 - 6/30/2022), all D11 health plan participants and covered spouses, need to complete the steps outlined below. Mojo Points are assigned to each step and the employee and covered spouse will need 400 Mojo points each (total of 800 points for employee + spouse coverage) to qualify.

    Complete your choice of the following activities to earn a total of 400 points

    Individual results of this process are completely confidential. BEST Health Plan and District 11 will only see an aggregate report and will not have access to individual results.


    HOW DO I KNOW THAT I HAVE QUALIFIED FOR THE PLATINUM PLAN?

    • If you are the employee, you must have a total of 400 Mojo point. 
    • If you are a spouse on the Health Plan, you must have 400 Mojo points. 
    • Once complete, you will receive an email from Mediterranean Wellness. 

     


     FAQ’s:

    If you had your annual physical and biometric screening retroactively back to 4/1/20, you can use those values for the purposes of this program. Simply log in to your Health Promotion Program portal to enter your data for the assigned Mojo points.

    Our plan allows you to have one routine annual physical and preventative biometric screening each plan year (7/1-6/30), so you are not required to wait 365 days in between appointments as long as it falls within a new plan year.

    Your spouse (if covered on your health plan) will have his/her own individual username and password. If you are using the same computer to enter your results and their results, please ensure you LOG OUT of your account before your spouse logs in with their individual username/password.

    In order for you to qualify for the platinum plan for next plan year, you and your covered spouse much each earn 400 Mojo points.

    HOW DO I LOG IN?

    **If you completed the Health Promotion Program last plan year, you will log in using the username and password you created. if this is your first time logging in, use the information below: 

    Username: Individual Member ID # found on your health Plan card

    Password:  6-digit date of birth (MMDDYY)

    **The employee and spouse will have thier own SEPERATE accounts**

    IMPORTANT DATES

    March 1, 2021: Comprehensive Nurse Evaluation (the call is due by 3/1/21, but the assigned activities are due May 15, 2021)

    May 15, 2021: Routine Annual Physical Exam

    May 15, 2021: Biometric Screening

    May 15, 2021: On-line Health Survey

    May 15, 2021: Nurse Assigned Activities to be completed
